Why do newspapers say things like "clashes that left four dead"? What does this mean? Clashes between who, and how were the people killed?

It reminds me of last year's cartoon riots, and news reports about how 'Muslim cartoon fury claims lives'. I mean what, those Muslims (as ever) were so angry (as ever) that they just... exploded with anger? Killed each other?

In this particular story you reach the SIXTEENTH paragraph before you learn that "three people died when police fired on protesters after a police station came under attack" and that "a shoot-out with police left two protesters dead, and six police officers injured"
